| Active Health Plans is committed to protecting the information we
receive from you. We follow reasonable technical and management practices to
help protect the confidentiality, security and integrity of data stored on our
system. While no computer system is completely secure, we believe the measures
implemented by our web site reduce the likelihood of security problems to a
level appropriate to the type of data involved. Our site encrypts your personal information using secure socket layer (SSL)
technology to provide for the secure transmission of the information from your
PC to our servers. In addition, only those employees who need access to your
information in order to perform their duties are allowed such access. Any

Enrollment Information
The enrollment information we collect may be disclosed to our staff
and to third parties involved in the completion of your enrollment into
the Active Health Plans program. We may use your email address or phone number
to contact you if we have
questions about your enrollment form. We may also use your email address to notify you
about new services or special programs, or send you
information if you have opted-in. Emails are sent only to customers who have
chosen to receive them (opted-in) or who have enrolled into our program.
At any time, you may stop receiving these emails (opt out) if you desire.
Data Collection and Reporting Service
This site makes use of a technology designed to enhance your experience and provide analytics. The technology, provided as a hosted service
application by Google and Atlasonepoint, which provides aggregate reporting
information to the site owner only. No personal or personally identifiable
information is gathered or used for this process.
Server Logs
In an effort to make our site as effective and enjoyable as possible, the
computers that operate the site will collect certain information each time you
visit the site and store these statistics in server logs. These statistics do
not identify you personally, but will give us information regarding the type of
user who is accessing the site and certain browsing activities of that user.
Information collected in this way includes the IP address of the user accessing
our site (i.e. the unique I.D. number of the user's computer), the type of

Active Health Plans may use various outside agencies (third party service providers)

assist in the enrollment process. Some of these third parties may need access
to your information in order to make the services provided through our website

connection with our website.
We may use third party advertising companies to serve ads on our behalf. These

anonymous.

From time to time, Active Health Plans may enter into strategic alliances to enhance

with Active Health Plans (meaning it would have both the Active Health Plans logo and the
logo or trademarks of another company), you should be aware that these sites'
